DOT Physical Examinations

 

Dr. Bantock is a Certified Medical Examiner for the Federal Motor Carrier Safety Administration.  Depending on your health you are able to be certified for a maximum of 2 years.  He performs DOT physical exams for commercial drivers, uber and lyft drivers, and employers who require an employee to have a DOT examination.

Forms

 

If you are able to, please fill out the driver portion of the DOT Physical Exam Forms prior to your visit.  Please only fill out the driver portion.

 

The DOT Physical Exam includes:

 

  • – Health history review
  • – Review of labs and other pertinent results if necessary
  • – Thorough physical exam
  • – Urine test

 

What To Bring to your DOT Physical

 

If you have information that needs to be reviewed by Dr. Bantock in order to pass your DOT physical exam such as results from your diabetes specialist, cardiologist, primary care physician, eye doctor, sleep doctor, neurologist, and Skilled Performance Examination please bring them with you.  Please bring a list of your current medications as well. This will prevent delays in receiving your medical card that day.

 

Please avoid drinking coffee, smoking, and stressful situations prior to your visit as it may unnecessarily raise your blood pressure and affect the results of your exam.
